For your convenience, we will gladly file your insurance forms electronically, at no charge. This enables a faster response from your insurance carrier. Typically, we will recieve your EOB (explanation of benefits) in 10-14 business days. If after 30 days we have not recieved an EOB, then it will be the patient's responsibility to pay the remaining balance. We are an in-network provider with those companies listed below. All other insurance is considered out-of-network; therefore we collect your portion of all treatment on the day of service.
